PURPOSE OF THIS QUESTIONNAIRE
This questionnaire is a required part of the Mold K9 Inspectors LLC pre-inspection process. It is designed to document historical and current moisture conditions, building characteristics, and environmental factors that may contribute to microbial amplification within a structure.
The information provided assists our inspection team and certified canine mold detection dog, Jett, in:
  • Identifying potential moisture-driven problem areas
  • Guiding canine detection routes and focus zones
  • Interpreting canine alerts within the proper building context
  • Determining whether additional investigation, moisture correction, or testing may be recommended
This process follows industry-recognized best practices consistent with NAMP and NORMI standards.
